Well I may just say from our stats:

1. We have many sites which has our banner located above the fold and they has low CTR while other sites which has banner located much lower has 2x times more clicks.
That's why it's not question to locate above the fold or below. Everything depends on the site.

For example on many sites in order to play game you need to visit two pages. On first one webmasters place banners and link to game. People who visit that site already know where is link located. And never stay on this page long. So those impressions are useless for banner network.

And there are many examples for making more impressions. I hope all know about them.

And on those pages you may place banner on any place but it won't be ever clicked.

2. For sure amount of banner networks on one page has huge impact. That's why "quality" sites never use more than 3-4 networks and place banners in different places.
And again this is what I mean under quality term.

3. Amount of visitors almost has no impact on CTR.
Again from our stats I see many small sites which has CTR 3% and many big sites with CTR lower than 1.5%
If you just buy impressions from warez sites, popunders etc of course you have a lot of visitors but neither of them even see your site. That traffic used only for Alexa rank and having more impressions. Nothing more.

In my opinion low CTR is because of following factors:
- poor banners used
- banners located in places where nobody see/click them (e.x. too far from main content, proxy pages)
- untargeted traffic

I even consider a "quality" site to have lower CTR, just because of the fact that the visitors of a quality site feel less need to leave the page (click on a banner).
